do not let these 10 forms overwhelm you!! Most
forms will not apply to you. There are 10 forms all together.
I have added a description of each form for you
so you can decide if you need to fill out the form before
opening it up.
You can print out the pdf form and mail or fax it back
to me. You can also download the Word document and enter
data
on the form and e-mail it back to me. I am open to anyway
this process can be made easier for you. My fax
number is (858) 534-7840 if you want to fax it back. I
need to retype your forms into the national database so
please
keep it legible!!
These forms apply to any California Space Grant
Funding you received from February 1, 2005 to January 31,
2006. I need all of your
forms turned in to me by February 20th, 2006.
It
is important that any funds that you have received from
us
are accounted on these forms ONE time only. You can
list the program on other forms, so if you received
$500 dollars from us, then all the forms combined must equal
$500. The exception to this is the Applicant
Awardee form.
List the student award amount on the form, and then
once again on the Fellowship/Scholarship form list the total
program cost.

Applicant/Awardee
Form - Fill out one form for
each student who applied to your program. This includes
students that initially showed an interest. We
can not just include students who received scholarships or
it looks as if we give money to every student that walks into
our offices. Please provide a few forms with names of students
that applied or showed an interest to your program, but did
not receive funding. Each student must have a project
that they were (or wanted to) work on which will be described
in the next form - the Fellowship/Scholarship Form. (For
each project listed on the Applicant/Awardee Form,
we need the form below filled out).

that have built up facilities and capabilities involving laboratories,
mission control centers, etc. that are used for both educational
space research.
